    SUMMARY

    Provide nursing care to clinic patients under supervision of Clinic Manager. Primarily work as a float nurse for Provider with rotating weekends & holidays when clinic is open. One evening covering late clinic per week.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

    Perform selected procedures and therapeutic treatments within his/her level of competencies according to policies and procedures.

    Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.

    Prepare, administer and record selected oral, subcutaneous and intramuscular medications.

    Observe, record, and report to provider any significant change in the patient's condition associated with drugs, treatments, or any other disease process.

    Ensure that clinic protocols are followed on all clinic patients (i.e. vital signs, diabetic foot care, etc.)

    Responsible for ordering, receiving and handling supplies as needed.

    Contact patients to provide information as needed or requested.

    Perform required recordkeeping for the Nebraska State Immunization Information System (NESIIS).

    Ensure that sample medications are kept in a safe and secure environment.

    Uphold confidentiality and HIPAA compliance at all times.

    Other duties as assigned by the Supervisor.

    EDUCATION and/or EXPERIENCE

    One year clinic experience desired

    CERTIFICATES, LICENSES, REGISTRATIONS

    Radiologic Technologists will perform departmental procedures, and assigned activities by the Manager of Imaging Services.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Full or part time employment as designated and on call as necessary with the rotation schedule of fellow staff members.
    Knowledge of departmental equipment and procedures pertaining to Radiology and Cardiology.
    Knowledge of State and Federal regulations and requirements for Radiologic Technologists.
    Knowledge of all required responsibilities from an accredited Radiologic technology educational program pertaining to physics, anatomy, physiology, medical terminology, radiation protection, radiation safety, body positioning, knowledge of technical exposure factors, and compliance with The American Society of Radiologic Technologist Code of Ethics.
    Adhere to staff scheduling determined by Imaging Services Manager.

    Participate in all safety, security and infection control programs that are mandatory as well as those required and provided by the department.

    Uphold confidentiality and HIPAA compliance at all times and report violations to the Privacy Officer.

    Understand and comply with the requirements of the Saunders Medical Center's Compliance Program, including, but not limited to the Code of Conduct, the Compliance Policy, all Saunders Medical Center-wide policies for compliance, and compliance plans affecting specific duties and responsibilities.

    Audit accounts and charges to ensure accuracy, prepare QA reports.

    Answer inquiries from patients regarding account charges, insurance payments and balances due.

    Works with third party payers/patients to assure timely payment on accounts.

    Assist with special projects as assigned.

    Back up other positions within department when vacations, medical leave, absences, etc. occurs.

    Maintain skills/competencies required for this position.

    SUMMARY

    The Care Coordinator serves to support the patient and other care team members with population health management. This includes but is not limited to completing the scheduling of referrals, diagnostic tests, data tracking, reporting and directing patient communications.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

    Knowledge of population health management strategies and information systems for chronic disease management.

    Connect and coordinate patients with consultant services external and internal resources as directed by the care team.

    Schedule medical referrals, diagnostic tests and treatments following protocols ordered by provider. Assure patients are notified, understand instructions and complete all documentation in EMR and/or required by accepting provider/agency.

    Completes tracking of referrals to ensure receipt of results and reports.

    Ensures patient notification is completed and follows up with patients as appropriate.

    Tracks treatment room orders to ensure prior authorization is complete.

    Ensures treatment room medication and supplies needed for patient appointments are received.

    Assist patients in obtaining appointments for referrals.

    Maintain a current knowledge of community resources and maintains positive working relationships with those providing services and share resource knowledge with care team members.

    Knowledge of prior authorizations process and provide back-up assistance for prior authorizations as needed.

    Participate in Quality Assurance projects as assigned.

    Attend department and organizational staff meetings as scheduled.
