The Job
Enhance seniors, ability to manage their health and live independently by conducting initial home assessments and annual home reassessments to determine Nursing Facility Level of Care (NFLOC) status for Medi-Medi members, in conjunction with an RN. Satisfies requirements for SCAN state contract for NFLOC members and CMS requirements for Special Needs Plan (SNP) membership.
You Will
Perform initial and annual home assessments for Medi-Medi members referred to Independent Living Power (ILP) to determine eligibility Long-Term Services and Supports (LTSS).
Assist Registered Nurse in determining Nursing Facility Level of Care. Assesses for unmet LTSS needs, as well as need for complex case management referrals.
Provide short term care coordination/care management which includes coaching, education, navigation and referrals to community based resources.
Counsel patients to take an active role in both chronic disease and health prevention, utilizing self-management support and patient education.
Prepare Service Authorizations, arranges, coordinates, and monitors services for members per department guidelines.
Complete timely and accurate documentation in multiple computer systems to complete assessment and corresponding documentation: care plans, service plans, authorizations, and progress notes as necessary.
Evaluate and analyze assessment information to identify problems, goals and interventions to develop an individualized care plan based on SCAN's evidence-based clinical guidelines while exercising clinical knowledge. Mails care plans to members and physicians.
Follow SNP policy and procedures and ensures all appropriate assessments for this population are completed and acted upon within established timeframes.
Other duties as assigned.

Knowledge of NCQA standards, CMS and DHCS regulations.
Travel 50 % of the time in the assigned area (North San Diego County)- Must have a valid driver's license, automobile insurance and reliable transportation.
Your Qualifications
Bachelor's degree required.
3 years of experience in assessing the needs of older adults, preferably in a healthcare environment.
Home visits experience preferred.
Passionate about working with older adults.
Strong working knowledge of Medicare/Medicaid.
Strong knowledge of local community resources.
Working knowledge of medical terminology and abbreviations.
Proficient in MS Office.
BILINGUAL- Strongly preferred bilingual in English/Spanish and/or English/Chinese. (Test will be administered to assess proficiency.)
Travel 50 % of the time in the assigned area- Must have a valid driver's license, automobile insurance and reliable transportation.
