Department: Health Department
Pay: $14.66 per hour
Work Schedule: 20-30 hrs per week, between 8am-6pm, some evenings and weekends possible

This is a part-time, temporary position in the Sedgwick County Health Department for COVID-19 response. Grant funding for this position is through a grant that ends December 2022. The Care Resource Coordinator works with clients who are COVID-19 positive and clients that need to be quarantined to ensure they have the information and resources available to successfully self-isolate or self-quarantine without disease spread. Clients include refugees, migrants, recent immigrants, and persons for whom English is not a primary language. The Care Resource Coordinator improves access to testing and acts as a bridge between clients and key service providers, assisting clients in identifying services available to meet their needs. The Coordinator must communicate in a culturally and linguistically appropriate manner and must be viewed as a trusted resource to the communities. The Coordinator ensures patient confidentiality and may wear personal protective equipment (PPE) during the course of their work.

Perform assessment of client needs to safely isolate or quarantine in the home
Work with Disease Investigator to review case notes prior to meeting with client
Arrange for interpreter services as needed
Following protocol, meet with client and administer a basic health screening and schedule testing, if needed. Collect demographic and living situation information.
Address client’s questions and concerns, including about test results and exposures
Create and provide a basic resource kit to client
Provide community resource and service referrals, including Health Department and wrap-around services
As needed, work with Health Department staff and external partners for transportation to testing and/or placement into hotel/motel isolation/quarantine program
Follow HIPAA policies

Perform client services in a culturally appropriate manner
Provide interpretation services at testing and vaccine clinics
Drive to testing and vaccine clinics
Serve as a point of contact for families and stakeholders
Provide COVID-19 testing information and ensure access to testing
Educate residents about the importance of following health recommendations and treatment plans, specifically isolation and quarantine protocols and other prevention measures
Follow-up with clients daily
For each client, provide ongoing informal counseling and social support until their health status improves
Provide coaching and training sessions for individual clients, families, and groups
Maintain record of each client’s health status. Document compliance with recommendations and treatment plans.
Document activities and complete standard reports for the Care Resource Team

Engage community leadership and community-based organizations
Maintain an up-to-date inventory of client resources
Utilize electronic community referral system

Minimum Qualifications: High school diploma, GED or equivalent. One year of experience working or living in a multicultural setting. Proficiency in English and a language other than English. Proficiency is demonstrated by passing a standardized test. Per Sedgwick County policy, this is a driving level position that requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance. Preferred Qualifications: Associate’s or higher degree in a health or related field. One year health education experience. One year experience working as a community health worker or health navigator. Two years of experience working or living in a multicultural setting. One year of work experience in a public health department or healthcare setting. Completion of Community Health Worker training.
