What are the responsibilities and job description for the Community Hab/DSP (Scotia) position at Self Direction Partners?
After Work 2 or 3x a week- 5pm start time
Weekends (Flexable)- Morning or Afternoon shift
Goal: Independent living skills, assistance with personal errands and activities of daily living
Sign language is Preferred
The community hab worker/DSP will provide one to one, personalized supports to individual(s) with an intellectual or developmental disability according to the service plan designed by the individual receiving services. Services are provided individually within the community and/or at home, rather than within the structure of a group program. Due to the independent nature of this position, it is essential that staff have strong communication skills to provide on-going feedback, and be self-motivated.
The Community hab worker/DSP will assist individuals with specific valued outcomes as noted in their service plan; including, but not limited to, life enrichment, skills development related to employment opportunities, overall personal growth and independent living skills, as well as community participation and inclusion. Duties may also include assistance with personal errands, activities of daily living, recreation activities and public transportation travel training.
Qualifications:
- Valid NYS driver’s license and reliable vehicle.
- High School Diploma or GED may be required.
- Smartphone and/or tablet for electronic record keeping.
- Ability to work independently, demonstrate sound judgement, and complete necessary documentation.
Job Type: Part-time
Pay: $25.00 per hour
Expected hours: 5 – 15 per week
