What are the responsibilities and job description for the Pharmacist Clinical position at Self Regional Healthcare?
The Internal Medicine Clinical Pharmacist is responsible for the provision of comprehensive clinical pharmacy services for patients on the hospitalist service, including pharmacokinetic and therapeutic consultation. He or she provides review of medications prior to admission, works collaboratively with other clinicians and providers to implement and maintain innovative disease management programs, participates in the development of medication use management programs including clinical guidelines, critical pathways, disease management, and drug use programs. He or she participates in providing didactic and experiential training in Clinical Pharmacy for doctoral (PharmD) students and residents, as well as participating in clinical research projects.
Required: PharmD, and must be licensed as a pharmacist in South Carolina, or eligible
Preferred: Advanced training (residency, fellowship, and/or board certification), Internal Medicine Pharmacy experience
