Plan, develop and implement assigned outreach programs for Sentara. Work with stakeholders in the community to increase health insurance access by identifying potentially eligible uninsured and underinsured pregnant women and children, raise public awareness for pediatric outreach campaigns and provide comprehensive outreach assistance. Provide education and advocacy to identified families to determine eligibility for state health coverage programs and other community resources. Assist families with the application and enrollment process for the programs. Coordinate special projects and pediatric outreach initiatives as necessary to promote health onsite and within the community. Perform data collection and metrics reporting.Solid level of understanding regarding various insurance carriers, particularly FAMIS and Medicaid. Ability to understand local support agencies; such as Departments of Social Services, Health, etc.). Function independently and/or as part of a team. Detailed oriented with ability to focus on detailed paperwork. Organization skills. Ability to relate to people from a variety of socio-economic and ethnic backgrounds. Flexibility to work occasional weekends or evening hours. May participate in home visitation with families. Possess a valid State Driver's license. Have own means of reliable transportation and willing to travel within the region.
