We are currently searching for a talented professional to fill the position of Family Partner in the YOU, Inc Milford, MA area to support and coach parents on navigating through the children's mental health system in our Community Based Services Component.    

The Family Partner will be responsible for providing parents/caregivers with structured, one-to-one family support and training services that are aimed at addressing the needs of the identified youth client. The Family Partner is responsible for supporting the team or the clinicians in implementing the client’s care plan. This includes service eligibility determination, services plan development, service coordination, resource development, as well as provide education, support, coaching, training and advocacy. The Family Partner should attend to risk issues, documentation of care, timeliness of documentation, professionalism, caseload expectations, and administrative issues related to providing services that are aligned with best practices such as the Principles of Wrap-Around Care. 

**New Hire Sign-On Bonus: $2,500 (50% paid after 6 months, 50% paid after 12 months of continuous employment.)

*$250 paid towards student loans per year completed.

Responsibilities

• Assist the parent/caretaker with learning how to network and link to community resources and services that will support them in caring for their children.
• Assist the parent/caretaker in assessing the youth and families' needs with regard to education, support, coaching, modeling, guidance and understanding.
• Actively collaborate with schools, courts, state agencies and service providers with parental consent.
• Maintain regular, face-to-face contact with the family as determined by the Care Planning Team and maintain regular contact with treatment providers.

Qualifications

• High School Diploma or HiSET required.
• BA/BS in a human services, psychology, or related field preferred.
• Must be the parent or guardian of a child with a mental health diagnosis or disability
• Two years of experience working with children/adolescents/transition age youth.
• The Family Partner will be required to successfully complete skill and competency-based training in the delivery of Intensive Care Coordination consistent with the System of Care  philosophy and Wraparound Planning Process.
• Must have a valid driver's license.
• Bi-lingual in Spanish/English preferred.
