Overview:
This is a direct service position providing individual medical support and education to individuals in assigned programs.
To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals.
Hours assigned by supervisor; on call responsibilities.
Responsibilities:
(A) Meet with program Residence Director and Area Director for supervision and direction as needed to ensure appropriate medical health care is provided for participants.
(B) Attend staff meetings and participate in discussions of current medical/health issues; Residential topics; staff trainings – supportive devices; equipment; and any other medical house related issues.
(C) Attend other designated mandatory in-services.
Valid Driver’s License required.

- Assess and coordinate healthcare services for participants. Institute appropriate follow up and communicate all medical issues to: Physicians, nurse practitioners, and other medical licensed providers, Residence Director, Area Director, clinicians, and any other appropriate SHNC staff.
- Assist and transport individuals in designated programs with their medical appointments as recommended by their physicians and DDS standards.
- Document all nursing interventions and changes affecting the consumer in appropriate areas –EHR, progress notes, and staff communication log.
- Review ISPs for current knowledge of programming in self-medication, behavior plans, hygiene skills, and physical therapy plans.
- Maintain current medication and treatment orders according to MAP standards noting any side effects/adverse effects and report to appropriate people on the participant’s team.
- Maintain health records according to DDS standards assuring that necessary documentation is up to date.
Ensure that information in communicated to staff-residential and day program regarding recommendations and treatments.
- Collaborate with the Residential Team to ensure participant safety.

- To implement procedures and services including medical interventions, or additional duties, as assigned by the AVP/Director of Nursing.
LPN or RN with current MA license.
Experience working with people with acquired brain injuries and psychiatric disorders preferred.
