MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
Adult Day Health is seeking a Case Manager/Care Manager/Program Manager to assess each participant’s needs and assess the cost of care by preparing a plan of care and identifying and implementing services to meet those needs.
Responsibilities May Include But Are Not Limited To Provide case management services including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families.
Gather social history to assess family strengths and weaknesses, and existing problems.
Conduct home visits and office-based services, and accompany participants being served as needed.
Coordinate care with outreach workers, health educators, nutritionists, and other service providers
Identify and procure group services appropriate for the participant to maximize the individual’s ability to live outside an institution.
Monitor the delivery of services in coordination with the participant and third party service providers.
Maintain up-to-date case records with the status of case activity, including progress notes on all contacts.
QualificationsBachelor’s degree required; Master’s preferred in a health or human services field
At least 1 year of experience in a health or human services field
LSW preferred
If licensed as a Registered Nurse – must have at least 2 years of experience as a professional nurse in the field of aging or disabilities
