MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs.
This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes.
Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
Sevita is looking for a dedicated Program Assistant for Living Well Adult Day in West Boylston. This is a Monday- Friday dayshift opportunity offering full time hours.
Bilingual in Spanish is preferred.

- Provide assistance in the daily care of health impaired older adults or individuals in our care with brain injuries, intellectual / developmental disabilities, emotional or behavioral disability or mental diagnosis.
- Assist with the development and implementation of individual and group activity programs, and responsible for the care, safety, security, and social / recreation activities for the individuals receiving support.
- Assist individuals receiving care with daily living, including personal hygiene, bathing, bathroom assistance, grooming, preparing meals, and medication administration.
- Provide guidance with social skills, advocacy and self-esteem and teach strategies to live as independently as possible.
- Transport to medical appointments, errands, shopping or activities in the community.
Qualifications :
- High school diploma or its equivalent and must be 18 years of age
- Minimum one year experience in providing services to individuals with intellectual and developmental disabilities, preferably with older adults
- Physical in the past 12 months and annual TB test
- Proficient in vital signs, weights, Activities of Daily Living (ADLs) and independent ADLs
- Competence in CPR and First Aid
- Strong attention to detail and effective communication skills to manage relationships
- A reliable, responsible attitude and a compassionate approach
