SUMMARY

Under the clinical and managerial supervision of the Behavioral Health Manager, this position is responsible for carrying out duties related to the Behavioral Health services in both a community health department and community setting. Activities may focus on the individual and/or community-based health issue and emphasizes resilience and recovery.

ESSENTIAL FUNCTIONS

Works with the Community Health Department Director and Behavioral Health Manager to conduct healthy lifestyle goals regarding nutrition, physical fitness and the home environment with patients and their family members.

Assesses eligibility, assists clients in accessing services and collaborates with agency professionals in case planning and service coordination.

Uses sound judgment, conforms to adopted policies and procedures, solves problems and achieves results consistent with stated goals and objectives.

Provide one-on-one education with patients in their homes or in the clinic to improve self-care and management of their health condition.

Work with patients to identify and address barriers to enhance compliance and with families of patients to increase support for patient self-care.

Assess home environment and educate patients to mitigate health and safety risks and recruit participants for education and related services.

Inform patients of community services/resources, make referrals and follow-up on referrals.

Maintain confidentiality regarding all patient information.

Enter patient encounter information in the electronic medical records system and collect data for evaluation and quality improvement purposes.

Participate in training to increase knowledge and skills.

Participate in team meetings and prepare reports as required.

Work closely with patient healthcare teams.

Provide client transportation as needed.

Seeks consultation and collaboration when appropriate and or mandated.

Practice within the scope of education.

PERIPHERAL FUNCTIONS

Provides backup professional services within the scope of practice, for any of the personal health services provided by the Community Health Department.

Performs other related duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of chronic diseases, including mental health and substance abuse disorders, their management and prevention.

Knowledge of the community and understanding of humility when working with others.

Knowledge and familiarity with health promotion and disease prevention strategies.

Knowledge and adherence to HIPAA as it specifically pertains to all Community Health programs.

Knowledge of effective organization and prioritization of multiple program functions.

Skill in record keeping, organization and prioritization functions.

Skill in recognizing chronic diseases and the management and prevention of such.

Skill in maintaining confidentially.

Skill in problem solving and using sound judgment.

Ability to provide patient education including classes in the various settings, and the home.

Ability to provide excellent communication skills in English, with preference for bilingual Spanish/English.

Ability to use a computer for basic word processing and documentation in electronic medical records.

Ability to work with multicultural, multi-language and disenfranchised populations.

Ability to solve problems using sound professional judgment.

Ability to maintain confidentiality

EDUCATION AND EXPERIENCE

High School diploma, GED or equivalent, preferred.

Minimum of two years’ experience in health and or social services.

Sufficient fluency in English required to participate in team meetings in English and document patient encounters in English in the electronic medical record.

LICENSES/CERTIFICATES

Valid driver’s license in state of residence.

SPECIAL REQUIREMENTS

Submit to a criminal background check, as required by Washington State Law.

WORKING CONDITIONS

Work is performed in an office environment, clinical environment, client homes, and in the field. May be exposed to individuals or groups with potential for violent behavior; threats to the public (e.g. bioterrorism); and potential exposure to vaccines, and other hazardous chemical and biological materials. May be exposed to communicable and infectious diseases. May be assigned to respond and work a public health event, outbreak, emergency or intervention.

PHYSICAL REQUIREMENTS

The duties of the above position require standing for extended periods of time, sitting, walking, stooping, bending, reaching, pulling, twisting, and the ability to lift up to 25 pounds. Requires finger dexterity, sense of touch, gripping with fingers and hands; ability to see, hear voice conversation, and to speak.
