Jacksonville Healthcare Center has an opening for a RN/LVN Clinical Case Manager!
POSITION SUMMARY: To manage the process of assessment, planning, facilitation, coordination, evaluation, and advocacy for services and options to meet the individual resident needs to promote quality, cost-effective outcomes while adhering to Federal and State guidelines, and in accordance with company’s policies and procedures.
ESSENTIAL JOB FUNCTIONS:
Manages the RAI Process from resident admission to discharge in order to maintain clinical compliance and receive appropriate funding from Medicare, Medicaid and Managed Care pay sources
Completes MDS and all pertinent forms required in a timely, accurate, documentation-supported and case mix optimized manner Develops, with input from the interdisciplinary team members, individualized care plans reflective of the resident status
Coordinates resident assessments and services by other clinical departments
In conjunction with the Business Office Manager, provides timely information for all managed care clinical authorization and justifications
Completes the daily, weekly and monthly assigned duties as outlined on the “Role of the Clinical Case Manager” procedure form
Provides training and support to nurses and nurse aides on Activities of Daily Living (ADL) and other duties related to clinical case management
Manages the daily PPS/OBRA case management meetings and weekly level of care meetings
Attends daily start up, stand up, and weekly standards of care meetings in order to identify resident changes that impact the MDS and/or Care Plan
Participates in weekly standards of care meetings, monthly quality assurance and performance improvement, and triple check meetings
Attend training, meetings and seminars when necessary
Other duties as assigned
Requirements
SUMMARY OF QUALIFICATIONS:
Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State
Excellent communication skills to include written, verbal and the ability to read, as well as follow oral and written direction at a level necessary to accomplish the job
Minimum Data Set and Care Plan experience preferred
Must be capable of maintaining regular attendance and working long and irregular hours, including evenings and weekends as necessary
Salary : $64,000 - $81,000
