Job Posting for Clinical Assessment Coordinator at Smith Senior Living
Position Purpose: Markets to area hospitals, physicians and case managers; maintains existing accounts; develops new relationships; and conducts clinical field evaluations of client referrals for Smith Senior Living from referral sources.
Job Duties include:
Provides education and information to key hospital-based customers about programs and services SSL offers throughout the continuum of care.
Visits hospitals regularly and completes rounds as feasible in order to maintain relationships and secure consistent referrals.
Evaluates medical information to determine appropriate level of care, rehabilitation appropriateness, and medical stability in conjunction with medical staff and hospital policy.
Maintains knowledge of all programs offered by SSL and effectively promotes all Smith Senior Living entities.
Works through admissions staff to ensure proper coordination of planned transfer.
Informs discharges planners, internal and external and admissions staff of planned admission date.
Discusses admission requirements (i.e. clothing needs, insurance cards, visiting hours, equipment, etc.) with client/family as needed.
Proactively seeks out opportunities for meeting potential clients and families who may need services at all levels of care.
Conducts on-site clinical assessments at targeted hospitals as needed.
Attends all scheduled clinical and corporate meetings, which may be in the evening.
Works closely with Admissions and/or Director of Nursing, Administrator and other staff members as necessary.
Marketing Process, Effectively uses CRM systems and tools to manage referral sources/sales.
Maintains up to date customer profiles on existing referral sources.
Schedules and leads effective presentations/in-services
Education:
Degree in Nursing is preferred.
College degree in Marketing, Business or Allied Health field.
Experience:
Experience in Health Care Sales, Marketing, Insurance or related experience is preferred.
Knowledge of industry regulatory standards and compliance requirements.
