OBJECTIVE SUMMARY: Provides professional nursing care for hospital outpatients following established standards and practices. Assesses patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Serves as a role model, leader, and outpatient care coordinator of the multidisciplinary team providing patient care. Administrative duties may include scheduling appointments and communicating with patients via telephone or patient portal.

 

ESSENTIAL FUNCTIONS:

  • Does not directly manage or supervise, but works in coordination with clinic LPNs and MAs.
  • Interview patients to obtain current condition/complaint, measure vital signs, weight, and height.
  • Administers prescribed medications and treatments in accordance with nursing standards.
  • Prepares equipment and aids provider during treatment, examination, and testing of patients.
  • Maintains and reviews patient records, charts and other pertinent information. Reviews and reports tests and examination results.
  • Assesses and diagnoses patient and family needs to provide quality care to patients.
  • Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient.
  • Maintains up-to-date and accurate documentation of nursing care provided to facilitate the integration of information for use by the healthcare team to assure quality care.
  • Functions as a role model for current and new staff.
  • Attends all required safety training programs and can describe responsibilities related to general safety, department/service safety and specific job-related hazards.
  • Follows the hospital infection control policies.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees that promotes a professional, responsible and courteous environment.
  • Promotes effective working relations and works effectively as part of a department/unit team inter- and intra-departmentally to facilitate the departments/unit ability to meet its goals and objectives.
  • Directs other licenses and non-licensed personnel in the provision of care to patients as assigned.
  • Attends required meetings and participates in committees as requested.
  • Participates in professional development activities and maintains professional affiliations.
  • Maintains patient confidentiality.
  • Instructs patient and family regarding medications and treatment instructions. Health teaching regarding nutrition, exercise, preventative measures and tests, side effects of medications, normal vs. abnormal, etc.
  • Administration of medication via injection, blood draws, preparation of urine specimens, and complete lab requisitions.
  • Performs related work as required for several specialty providers including but not limited to Primary Care, Psychiatry, Behavioral Health, GI, Cardiology, Physiatry, Orthopedics, Geriatrics, or Sleep Medicine.
  • Perform transitional care management task such as; review of discharge instructions, medication reconciliation, care coordination, and appointment scheduling.
  • Perform chronic care management (CCM) tasks such as; follow care plans as directed by a provider and manage CCM database.
  • Perform Medicare Annual Wellness Visits as directed by clinic providers and CMS guidelines.
  • Perform Occupational Health and Medicine tasks as assigned.
  • Performs related work as assigned.

 

REQUIRED EDUCATION: Graduate of an accredited school of nursing
PREFERRED EDUCATION: BSN
REQUIRED EXPERIENCE: Six months of professional nursing experience required in a Clinic Setting
PREFERRED EXPERIENCE: One year of professional nursing experience in a clinic setting preferred
ADDITIONAL ELIGIBILITY QUALIFICATIONS: Washington State RN License, BLS

  • Knowledge of organizational policies, regulations and procedures to administer patient care
  • Knowledge of medical equipment and instruments to administer patient care
  • Knowledge of common safety hazards and precautions to establish a safe work environment
  • Skill in applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care
  • Skill in identifying problems and recommending solutions
  • Skill in preparing and maintaining records
  • Skill in developing and maintaining quality assurance
  • Skill in establishing and maintaining effective positive working relationships with patients, medical staff, and the public
  • Ability to handle multiple priorities
  • Ability to maintain quality control standards
  • Ability to react calmly and effectively in emergency situations
  • Ability to interpret, adapt, and apply guidelines and procedures
  • Independent discretion/decision making, critical thinking skills required
  • Ability to make decisions under pressure
  • Ability to work alone effectively as well as work with others

 

WORK ENVIRONMENT: Works in close proximity to others and/or in a distracting environment. Frequent exposure to communicable diseases, toxic substances, ionizing

radiation, medicinal preparations and other conditions common to a clinic or hospital environment.

 

PHYSICAL DEMANDS: Requires full range of body motion, manual and finger dexterity, and eye-hand coordination. Occasionally lifts and carries items weighing up to 50 lbs. Requires corrected vision and hearing to normal range.

Eligibility Requirements - Employees regularly scheduled at least 20 hours per week.

Employee Dependents - Your legal spouse or domestic partner, Dependent children until age 26.

Waiting Period - 1st of the month following 30 days from your date of hire.
