Medical Social Worker needed for FT/PT, temporary positions in Burlington, Camden & Gloucester Counties.
Interviews patients, families, and significant others, evaluating their present situation, providing support, and counseling, accordingly, referring to appropriate resources. Monitors the LOS (length of stay) of the patient to assist in timely discharge planning.
Conduct psychosocial assessments and provides crisis intervention, emotional support, and counseling services to patients. Collaborates with all members of the healthcare team, patient, and/or family to develop and implement a patient-centered plan of care. Obtains, interprets, and communicates necessary information regarding patients and families, including social history information to the healthcare team.
Refers patient/family to the hospital and community-based services to meet a patient continuum of care needs. Such interventions may include referral for financial psychiatric, rehabilitative, inpatient hospice, drug and alcohol treatment, and support groups. Documents the initial evaluation, as well as an ongoing activity when a significant event occurs. Interacts with co-workers, visitors, and other staff consistent with the core values of the hospital.
