Summary: The individual in this position serves the Council by administering the Intake process for determination of eligibility for our case management program and offering systems navigation to those referred to the case management program. Fluent in Spanish, Mandarin Chinese or Vietnamese.
Essential Duties & Responsibilities: The duties and responsibilities of this position shall include, but are not limited to the following. Other duties and responsibilities may be assigned as needed by the Council.
· Administering Intake process for case management referral by making initial contact, vetting, and assessment of potential clients.
· Provides referral support for clients who don’t qualify for case management service. Navigating social services and researching current community resources that will assist in making appropriate referrals.
· Develop working relationships with other providers and service agencies, especially those who work with a similar population.
· Liaison with Centro Rendu Case Management services, resource referrals to include helpline and Spanish Vincentian support, Food-bank walk-ins and volunteers
· Provides logistical /administrative support for case managers, including follow-up with current clients, and collection of data for evaluation of effectiveness of client service.
· Communicate diplomatically with various stakeholders, i.e., volunteers, clients, and fellow case managers.
· Be culturally and linguistically competent for the priority populations being served.
· Have a proven record in working with low-income communities of color.
· Possess and/or be willing to acquire engagement skills such as: Motivational interviewing, empathetic listening and responsiveness.
· Be organized, have coordination skills and pay attention to detail.
· Provides occasional case management as time allows.
· Completes data entry and reporting for various grant requirements.
· Works with the proper attitude that exemplifies the Society’s Statement of Values
Minimum Qualifications: In order to perform the essential duties and responsibilities of this position, an individual must posses the knowledge, training, skills and experience indicated by the following:
Education/Experience
BA in Social Services, Psychology, Education or related field; or 4 years’ equivalent experience in social human services to a diverse community, or a combination of education and /or training and/or work experience which provides the ability to perform the work of community connector.
Language Ability
This position requires excellent written and verbal communication skills, including ease in addressing groups of clients and communicating with diverse workgroups. Ability to read, analyze, and understand general business periodicals, journals, technical procedures, safety manuals, and regulations. Must have the ability to write reports and business correspondence. Bilingual in Spanish preferred.
