What are the responsibilities and job description for the Part Time Case Coordinator position at Sojourners Alliance?
Essential Duties & Responsibilities:
The Project Warmth Case Coordinator shall be responsible for:
- New client Intake and Orientation
- Developing short-term, intermediate, and long-term case management plans for all clients to include a plan to access appropriate mainstream resources and life skills assessment and training
- Overseeing progress toward short-term, intermediate, and long-term goals stated in case plans
- Meeting with all clients as needed, to provide assistance, assess progress and address problems
- Maintaining client case files and all Housing Program grant-specific documentation
- Providing monthly follow-up interviews when possible, by phone, mail, or in-person for the first six months after each resident has completed the program offered in their case management plan and moved out of the Sojourners' Alliance housing program.
- Coordinating with Contracted Counselor to provide mental health evaluations or services as needed
- Attending regular staff meetings to report on Housing Programs. The Project Warmth Case Coordinator will meet regularly with the Executive Director or Case Manager to discuss issues related to the Sojourners' Alliance Housing Programs
- Other duties assigned by Executive Director or Case Manager

Pay: From $17.00 per hour
