We are seeking a dedicated and detail-oriented Case Manager to join our team for the full school year. This full-time position involves coordinating and managing prehensive care plans for students with diverse needs. The ideal candidate will be mitted to supporting students and their families by providing resources and services that promote academic and personal success.
Key Responsibilities:
- Develop, implement, and monitor individualized care plans for students.
- Coordinate services and resources for students and their families.
- Collaborate with teachers, counselors, and other support staff to address student needs.
- Conduct assessments to identify student needs and progress.
- Maintain accurate and up-to-date records on student progress and services provided.
- municate regularly with students, families, and other stakeholders to ensure effective care coordination.
- Advocate for student needs within the school and munity.
- Provide crisis intervention and support as needed.

Educational Requirements:
- Bachelors degree in Social Work, Psychology, Counseling, or a related field (Masters degree preferred).
- Experience in case management, preferably in an educational or child-focused setting.
- Strong understanding of developmental, academic, and social-emotional needs of students.
California Certifications:
- Valid California Pupil Personnel Services (PPS) Credential.
- Certification in Social Work, School Counseling, or School Psychology preferred.
