Information, Outreach & Referral Specialist

FULL TIME 35 HOURS PER WEEK

We are seeking a well-organized experienced, team oriented social services professional to be a member of our Information Outreach and Referral Department.

RESPONSIBILITIES: provide telephone information and referral services to professionals, caregivers, elders and others from the community; assist with data base development and updates; complete intakes for agency programs and screen for service eligibility; participate in agency’s outreach efforts including public speaking and provide enhanced home based information and referral. Facilitate communication with/and responsible for the processing of e-referrals and phone referrals from local community health organization

QUALIFICATIONS: Bachelor’s Degree preferred or at least two years experience in social work, human services, psychology, sociology or related field; excellent written and verbal communication skills, experience working with a diverse population and outstanding assessment skills. Excellent computer skills required, including thorough knowledge of Microsoft Word, Outlook, Excel, live data entry experience and call center experience. Cantonese, Mandarin or Spanish speaking candidates are encouraged to apply. Must have a valid driver's license and provide own transportation.

SALARY: $44,460
