Weekly Hours: 40 hours/week: 25 hours of direct care & 15 hours of indirect work
Proposed Schedule: Sunday – Thursday: flexible hours between 8am - 8pm to meet the needs of families
Job Summary:
The Family Support Case Manager is a liaison between the Department of Developmental Services and the families we serve. The Family Support Case Manager will help develop a family support plan and complete assessments to determine the needs of an individual as the function within the family unit, both in the home and the community. You may consult with the Positive Behavioral Supports (PBS) Director to develop a plan for assessing individuals with behavior issues, collecting data, and devising a behavior plan for the individual and evaluating the effectiveness of the plan. Overall, the Family Support Case Manager will lead the implementation of the family support plan as well as assist the family in applying for MassHealth, Social Security, food stamps and other public benefits. The Family Support Case Manager will also attend meetings and advocate on behalf of the family.
Essential Job Functions:
· Provide case management to families through the Family Support Center including assessments, short term and extended navigation services, financial allocation support, activities, training and resource assistance.
· Provide direct support to the individual and family in the family home and/or community setting.
· Assist families in the development and implementation of Family Support Plans including; assessments, goals and objectives.
· Provide training to family members utilizing intervention strategies in home/community.
· Serve as liaison between family and the Department of Developmental Services as needed.
· Provide assistance in locating community resources and referrals as appropriate.
· Act as liaison between family and other service providers such as special education programs, health care providers and other community resources.
· Maintains timely and accurate documentation according to program and funding requirements.
· Facilitate various social and recreational groups for families through the Family Support Center or via Zoom as needed.
· Conduct outreach on a regular basis to develop community partnerships.
Required Knowledge, Skills and Abilities
· Must have a working knowledge of applied behavior analysis.
· Must have an understanding of behavioral characteristics associated with behavior disorders, autism, intellectual disabilities, traumatic brain injuries and other behavior issues.
· Has good communication skills, both written and verbal, particularly with parents, DDS and other providers.
· Must have excellent interpersonal skills.
· Must be able to handle emotionally charged situations.
· Has the ability to train other people to apply behavior management techniques effectively.
· Has knowledge of and knows how to work with various support systems that are available in the family’s community.
Qualifications: Education & Experience Required:
· High School Diploma or equivalent, bachelor’s degree in psychology, social work or human services preferred.
· Experience in Applied Behavioral Analysis.
· Understanding of supports and services needed by families in caring for disabled members.
· Three years’ experience working with individuals with developmental disabilities.
· Experience implementing behavior plans.
· Maintain CPR/First Aid certifications, Safety-Care training, agency-required trainings including online RELIAS trainings and those requested by the Director of Family Support/SSSS.
· Excellent listening and communication skills
· Proficient using Microsoft Office and able to learn other office computer software.
General Expectations:
· Must have a current, valid state issued driver’s license.
· Employees are subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration.
· Pass South Shore Support Services background and reference checks, and a DDS criminal background check.
