Summary:
The Acute Care Case Manager evaluates the patient's health status and patient/family/significant other's care capabilities to facilitate the discharge planning process. The case manager develops multi-disciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical review utilizing Interqual criteria for appropriate level of care and insurance coverage.
Qualifications:
· Bachelor's degree in a healthcare related field or equivalent experience.
· 3 – 5 years of case management related health care experience. A minimum of 7 years of experience is required without a Bachelor's degree.
· Care Coordination.
· Excellent written and verbal communication skills. Basic Microsoft Office.
· Able to achieve the following competencies within 90 days:
o Abuse reporting
o Guardianship
o Discharge planning/Case Coordination
o Referral to community resources
o Utilization review
