We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. Southwest Care Center is seeking a full-time Behavioral Health Counselor. The Behavioral Health Counsellor assesses patients for common mental and substance abuse disorders, provides patient education regarding treatment options, and collaborates with our multidisciplinary team including case managers, nurses, medical staff and other mental health providers as appropriate.
Practice Highlights:

  • Hours of Operation: Monday - Friday, typically 8a-5p
  • System-wide Athena EMR
  • Onsite care team includes Physicians, Nurse Practitioners, Clinical Pharmacists, Behavior Health Clinicians, Case Managers, RN Clinic Managers, RNs, Medical Assistants, and Diabetes Educators

Candidate Highlights:
Education: Master's Degree

  • Monitor clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management prescribed by PCPs, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
  • Provide brief counseling or psychotherapy using ev idence - based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
  • Provide or facilitate in - clinic or outside referrals to evidence - based psychosocial treatments (e.g, CBT, IPT, PST) as clinically indicated.
  • Conduct regularly scheduled caseload consultation with the multidisciplinary team and communicate resulting treatment recommendations to the patient’s PCP. These consultations will primarily focus on patients who are new to treatment or who are not improving as expected.
  • Facilitate patient engagement and follow - up in mental health care.
  • Track patient follow - up and clinical outcomes
  • Document patient progress and treatment recommendations
  • Facilitate treatment plan changes for patients who are not improving as expected i n consultation with the PCP and the psychiatric practitioner. These may include changes in medications or psychosocial treatments or appropriate referrals for additional services.
  • Facilitate referrals for clinically indicated services outside the primary c are clinic
  • Complete relapse prevention plans with clients with substance use disorders.
