Functions:
Responsible for the overall coordination and implementation of health care services within boundaries of registration.
Duties and Responsibilities:
- Provides, coordinates and documents direct nursing care. (Essential and Non-modifiable)
- Supervises the work of L.P.N.s, as directed. (Essential and Non-modifiable)
3. Ensures that person centered plans adhere to the policies, procedures, and conditions of
appropriate regulatory agencies, with an emphasis on health-related issues. Monitor all
records and reports for persons supported and any other data as determined by the
supervisor and/or conditions of appropriate regulatory agencies. (Essential and Non-
modifiable)
4. Assesses persons supported current and potential health care needs, urgent care and health care needs, providing appropriate intervention and access to services and consultation as necessary. Trains staff and/or family, as needed, in order to implement the service plan. Monitors and maintains recordsand reports as directed. (Essential and Non-modifiable)
5. Serve as liaison with families, guardians, other Sparc programs, and external agencies,
including the medical community on medical issues. (Essential and Non-modifiable)
6. Make sure the team members are updated from person centered planning
7. Performs other related duties as assigned.*

Skills Needed:
- Must be detail oriented with excellent time management skills and have a proven ability to meet various deadlines.
- Possess leadership skills necessary to effectively monitor, and oversee the program area and duties.
- Ability to effectively communicate with staff, community, families and individuals supported.
Education/Experience Requirements:
- One year nursing experience
- Basic Life Service certification
- A minimum of a current Illinois RN license, with experience in a related human service field preferred. A Bacalaureate is preferred.
Qualifications:
- Demonstration of the written and oral skills necessary to effectively monitor and oversee a health care plan.
- The demonstrated ability to effectively communicate with staff, community, families and persons served.
- Must have reliable transportation in order to monitor multiple sites. Must also meet the requirements of the agency insurance carrier and drive company vehicles, as needed.
Other Requirements:
- Experience with individuals with developmental disabilities preferred.
- Must have a current, valid Illinois Driver’s License.
- Must have current, valid proof of automobile insurance.
- Must have reliable transportation to travel to various local work sites.
- Ability to physically handle any special needs of individuals supported by Sparc, in order to assist program staff in emergencies, including the ability to lift and transfer individuals as needed.
- Attend and satisfactorily complete all training requirements, including Nurse-trainer course.
- Ability to work a flexible schedule as needed/required.
