Greetings! It is with great optimism that We wanted to share some information on our growing Primary Care and Long-Term Care practices. With a mutual focus and passion on providing excellent care to our patients and residents, both Dr. Roscoe and I believe you would be a great part of the team. Our current team of 18 providers is small enough where your voice is heard and yet, big enough to provide you a stable and effective platform to practice medicine in your hometown! Your skill set offers us all collectively the opportunity to springboard our goals. We find our providers are looking for a few key elements in their practice of medicine:

Our objective is to share some of the key components upon which a we can all agree upon and base the working relationship from. As part of the clinical team we will provide the infrastructure and staffing to support you and manage the following components of the Primary Care practice and the Spartan LTC program:
· Flexible scheduling for Long Term Care Providers
· Regional Nurse Coordinator to manage your schedule and facilitate your interaction with facilities
· Clinical protocols and Chronic Disease Guidelines to standardize care
· LTC Annual Wellness Team to help perform routine tasks on your behalf
· Chronic Care Management Team that follows your patients monthly to close gaps in care
· Dedicated Fax line and fax response by CRNP’s to save your evenings and weekends from reviewing faxes
· Dedicated calendar for you as a provider that you can access and manage remotely from your computer or smartphone on our proprietary Spartan Careshield Scheduling App
· Provision of a dedicated smartphone for each of your facilities to enable ease of communication that is quick and secure with nursing staff
The combined support and framework of the Spartan LTC program will allow you to focus and expand your presence and care within Long Term Care facilities. As a provider within the Primary Care office and/or the Long Term Care facilities we anticipate your key responsibilities will be:
Primary Care:
· Routine office visits/Chronic Disease Management
· Well child visits
· Acute office visits
· Adult Physicals and Annual Wellness Exams
· Procedures as provider requests
Medical Directorships:
· Attend monthly meetings (Quality and Medical Executive meetings)
· Attend other meetings as necessary (infection control, Re-hospitalization, State Audit Support etc.)
· Provide oversight and clinical direction in a manner that improves the quality of care for residents and supports the objectives of the facility
· On Call support reasonable and limited to phone call only
Long Term Care Provider:
· Acute visits
· Routine Visits
· Admissions/Discharges
· Limited After hours on call support
Compensation :WE can look at options such as:
1. Flat Rate per encounter compensation per encounter
2. Flat Rate per day of work
3. % of full time comp based on days per month worked
4. Full Time Physician role in the $200,000 to 300,000/yr range
a. Potential FTE ancillary benefits
· Base salary of $225,000 to $250,000 based upon experience
· 100 % of Malpractice Insurance
· 401k match up to 4% of total salary
· Health insurance 100% coverage for provider and family
· Bonuses available based upon productivity
· 15 days of Paid Vacation
· $5000 credit towards CME
