What are the responsibilities and job description for the Case Manager position at Sponsors, Inc.?

Organizational Purpose:

Sponsors seeks to improve outcomes and opportunities for individuals with conviction histories and to assist individuals releasing from incarceration in their successful reintegration to our community. Sponsors is nationally recognized as a model utilizing best practices in reentry services.

Purpose of Position:

Individualized case management is a core component of Sponsors' programs, and case managers serve a vital role in conducting assessments, initiating program referrals, developing dynamic case plans, and in carrying out the overall operations of the Transitional Housing program.

Position Reporting:

This position reports directly to the Transitions Program Director.

Supervisory Responsibilities:

It is possible that an individual in this position could supervise one or more interns.

Job Responsibilities

1. Oversee all aspects of Case Management for assigned program participants.

a. Perform screening and intake for incoming residents using program-approved assessment tools.

b. Monitor status of individualized case plans based on LS/CMI transitional assessment and/or reassessment as needed.

c. Initiate referrals; coordinate and communicate regularly with participants’ parole officers and treatment providers.

d. Monitor compliance with conditions of each participant’s release and all Sponsors program requirements.

e. Monitor compliance with requirements of other related programs, including cognitive groups, drug/alcohol treatment, parenting classes, and others as applicable.

f. Conduct regularly scheduled 1:1 sessions with each participant and maintain communication as necessary.

g. Perform scheduled and random drug and alcohol screenings

h. Utilize web-based programs and other systems to chart participant progress, maintain accurate entry/discharge records, and coordinate care.

i. Facilitate Sponsors cognitive groups and ensure program fidelity, participant compliance, and maintain all group related paperwork in the agency designated time frame.

2. Ensure program participants are compliant and progressing appropriately in transition/exit planning

a. Assist participants in developing transition plans with a goal toward long-term stable housing that meets the approval of assigned parole officers.

b. Monitor and discuss participants’ employment, savings, and financial management, particularly as it relates to their transition from Sponsors and into stable housing.

c. Maintain appropriate participant records, both electronic and physical, as required by Sponsors, collaborating agencies, and grant requirements.

3. Assist in the coordination of program operations.

a. Monitor common areas and individual housing units to ensure the safety, security, and cleanliness of the facility.

b. Receive and document physical donations (e.g., clothing items)

c. Receive and route calls, faxes, and walk-in program visitors.

4. Intra-Agency Collaboration and Communication

a. Attend and actively participate in scheduled program staff meetings, biweekly all-staff meetings, and weekly case management meetings.

b. Participate in scheduled trainings as requested by management staff.

c. Maintain regular direct communication, both written and verbal, with program supervisors and other Sponsors staff regarding participant progress and conduct, facilities operations, and organizational protocols/procedures.

Education and Experience:

  • Higher education degree in related field preferred.
  • 1 year of experience providing direct client services in social service setting

Work Environment:

The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties. This position will travel between locations at the direction of the supervisor in an effort to meet organizational needs. May encounter frequent interruptions throughout the work day.

Physical Demands:

Walking upstairs, sitting at a desk for prolong periods of time. Noise level in the work environment is typical of that of an office environment.

Pay: $15.30 - $18.70 per hour
