JOB TITLE: Nursing Supervisor
REPORTS TO: Clinical Manager
- BASIC PURPOSE
The Nursing Supervisor provides oversight to the nursing clinical division of the home health agency including oversight to the home health aides. This oversight must include the following: overseeing that nursing patient and personnel assignments are managed and coordinated, assuring that patient needs are continually assessed; and assuring the development, implementation, and updates of the individualized nursing plans of care are managed. Assists the Clinical Manager in implementing the safety and infection control program; ensures medical records are maintained appropriately; participates in clinical record review and chart audits and maintenance of medical supplies. Participates with the Clinical Manager in the Quality Assessment and Performance Improvement process and assures that all nurses and aides function according to state law, and Medicare/Medicaid standards for Home Health. The Nursing Supervisor reports to and receives consultation, direction and supervision from the Clinical Manager. In the absence of the Clinical Manager, the Nursing Supervisor is to fill the role of the Clinical Manager.
- PRIMARY RESPONSIBILITIES
Planning, Organizing & Implementation
- Provides direction, supervision and consultation to the nursing staff.
- Clinically manages and oversees patient care by performing the following functions:
- Making patient and personnel assignments;
- Coordinating patient care;
- Assuring that patient needs are continually assessed; and
- Assuring the development, implementation, and updates of the individualized plan of care.
- Serves as a resource to all nursing and personnel regarding clinical activities including management of physician orders, reviewing admission documentation including medications, documentation of plans of care, face-to-face, etc.
- Assists with case conferencing and team meetings to guarantee effective team communication and coordination.
- Assists the Clinical Manager with the Quality Assessment and Performance Improvement program including participation in clinical record review.
- Completes reports for Clinical Manager as required.
- Oversees the provision of nursing and home health aide services consistent with standards of practice, community need, agency policies and procedures, Medicare/Medicaid guidelines and State Home Health licensure laws.
- Develops written goals, objectives and activities for the delivery of home health services.
- Establishes the responsibilities and qualifications for clinical services positions.
- Develops clinical policies and procedures for the delivery of patient services.
- Incorporates Process Improvement principles into clinical service delivery.
- Participates in program planning, identifying new program areas, establishing policies and procedures related to service delivery and evaluation of agency programs.
- Supervises the provision of clinical care to patients and provides care on an
as-needed basis. - Works with clinical staff to assist as needed with difficult care issues and to alter patient plans of care.
- Advises, assists and collaborates with clinicians on an as-needed basis to assess patient situations, staff problems, and coordinates physician referrals.
- Ensures that each patient receives quality care consistent with agency policies based on patient need and state/federal guidelines.
- Utilizes clinical knowledge and skills in overseeing the provision of nursing care to home health patients on a high quality, consistent basis.
- Demonstrates basic keyboarding and computer skills and is able to use agency software.
- Provides and instructs nursing and aide staff orientation.
- Assists with the oversight of the Infection Control and Safety program.
- In absence of the Clinical Manager, the Nursing Supervisor assumes the role of the Clinical Manager in charge with the same responsibilities and obligations as the Clinical Manager.
Human Resource Management
- Assists with the recruiting, recommending for hire, and terminating home health nurses and home health aides and contract personnel.
- Assists Clinical Manager with determining staffing patterns that will accomplish objectives and promote maximum utilization of personnel.
- Helps identify staff development needs and ensures that inservices are held and continuing education opportunities for staff are sought and encouraged.
- Conducts employee performance evaluations appraisals at the completion of the probationary period and annually thereafter within 30 days of the evaluation due date.
- Ensures accurate and timely completion of:
- Payroll Reports/Time Sheets
- Employee Incident/Accident Reports
- Required Personnel Forms
- Informs the Clinical Manager of patient care, staffing issues and/or personnel issues on a timely basis.
- Oversees that services are coordinated, and care is available through Home Health Care RNs 24 hours a day, 7 days per week.
- Ensures that an adequate stock of medical supplies needed for patient care provision is available for use by staff for patient care.
- Ensures that home health care nurses and aides receive supervision, via appropriate nursing staff.
- Guarantees that home health aides are supervised every 14 days per regulatory guidelines.
Fiscal Management
- Understands the need for staff and contractor productivity and effective utilization.
- Utilizes available data resources, such as reports for scheduling and supervising staff relative to assignments and visits.
- Participates in budget preparation and census/visit projections for clinical services.
- Evaluates services provided though patient outcomes, statistical and financial data, and other quality improvement activities.
- Assists in the investigation of patient and customer concerns and complaints.
- Ensures effective and efficient utilization of personnel in accordance with established productivity guidelines.
- Approves requests for time off, ensuring that there is coverage for nursing and aide positions/services.
- Manages office staff for smooth day-to-day operation and workflow of the home health agency.

- Maintains current knowledge of home health principles and practices.
- Maintains current knowledge of individual discipline practice acts, standards of practice, applicable laws, regulations, and practices.
- Develops and maintains cooperative working relationships with other home care agencies, professionals and referral sources in order to encourage the exchange of information and services and to upgrade the level of professionalism and quality of home care services.
- Attends external meetings as directed and presents information on agency services to outside sources.

- PHYSICAL REQUIREMENTS (See Attached Physical Requirements Form)
- This position requires the nurse to work in both an office and home environment. Position may require travel to other locations, meetings and seminars, etc. The position requires performing both patient and supervisory visits in a patients home that would be in a variety of environments (housing styles, cleaning and maintenance, heating and cooling).
- Visual/hearing ability must be sufficient to comprehend written and verbal communication and include excellent listening skills.
- Must be able to deal effectively with stress.
- EDUCATION AND EXPERIENCE
- Licensure and Education:
- A registered nurse licensed in the state of Ohio.
- Graduate of an accredited nursing program with Baccalaureate or masters degree in nursing or related field, preferred.
- Experience:
- Minimum of three (3) years clinical experience, with at least one (1) year in home health required.
- Minimum two (2) years in a supervisory or management position.
- Demonstrates basic keyboarding and computer skills and is able to use agency software.
- Possesses leadership skills, enjoys working with people, assertive, mature, approachable and open-minde
- Further Registration and/or Certification:
- Valid Ohio drivers license and automobile insurance, per state guidelines.
- Must have use of automobile in good working condition.
