What are the responsibilities and job description for the MCCN Care Coordinator position at Springwell?
Springwell is working with Massachusetts Care Coordination Network to facilitate the appropriate delivery of health care services and improve health outcomes. We are seeking a dynamic Care Coordinator to help MassHealth members navigate the community and connect them to services that will support in meeting the care goals identified.
POSITION DESCRIPTION:
The MCCN Care Coordinator works closely with MCCN to provide long-term services and supports to MassHealth members between the ages of 3 years and 64 years old. The Care Coordinator supports MH members in navigating the community by connecting them to services that will support in addressing identified care needs, including medical, social, legal, educational, and economic. Care Coordinators engage with their members through phone calls and in-home visits, completing Comprehensive Assessments, creating care plans, following up with collaterals and collaborating with the MCCN care team.
QUALIFICATIONS:
- Bachelor’s degree in social work, human services, nursing, psychology, sociology, or related field from an accredited college/university OR an Associate’s degree and at least one year professional experience in the field OR at least three years of relevant professional experience.
- Care Coordination and Behavioral Health experience preferred.
- Ability to communicate effectively verbally and in writing.
- Strong organization skills with attention to detail and ability to multi-task.
- Must have a reliable car, current driver's license and safe driving record.
