This is a full-time year-round nursing position providing care to adults with mental health conditions in a residential setting. Working with a multidisciplinary team, the Nurse will monitor and respond to adults' health and wellness needs and collaborate with internal and external providers to support residents' recovery process. Effectively engage, assess, monitor, educate, coordinate, and provide interventions for participants and families to improve quality of care and participant health outcomes. Provide participant, family/caregiver teaching and education about health and wellness, safety factors, self-management skills, medications, self-monitoring, action plans, long-term planning, when and how to access care. Conduct in-person visits with residents in the program to monitor health and wellness issues. Participate in group treatment and education interventions QUALIFICATIONS: Registered Nurse as defined by the Maine State Board of Nursing. Minimum Experience Requirements : One-year post-licensure experience in community health nursing, primary care, hospital, or psychiatric nursing preferred. Experience working with individuals with significant mental illness, serious emotional disturbance, and developmental disabilities. Must possess high level organizational and time management skills. Must be computer literate, able to generate presentations and present basic data summaries, able to navigate an electronic health record, and use HealthInfoNet.
