The Department of Economic Security, Division of Aging and Adult Services, seeks an experienced and highly motivated individual to join our team as a Care Coordinator. DAAS supports at-risk Arizonans to meet their basic needs and to live safely, with dignity and independence. Services and programs reach a diverse population of Arizonans.
This position provides a critical role in supporting clients with specialized needs and will serve as the single point of contact/liaison for APS investigators throughout the entire administration who need additional resources to support their clients, to improve client outcomes and prevent recurrence in APS. You will design protocols for referral, triage, and acceptance for additional one-time needs, such as hoarding clean-up, bed bug remediation, or bio-hazard cleaning. You will also serve as liaison with vendors to authorize service provision, track outcomes of clients for whom APS have provided these services and prepare reports and presentations around the effectiveness of the initiative, both in terms of data around recurrence and in terms of qualitative narrative.
This position may offer the ability to work remotely, within Arizona, based upon the department's business needs and continual meeting of expected performance measures.

Essential duties and responsibilities include but are not limited to:
• Serve as point of contact/liaison for APS investigators statewide who have cases/investigations that meet the specified criteria; triage whether additional services/costs will be incurred by or provided by APS.
• Create checklists and protocols for approving client expenditures, authorize services for clients and in coordination with the IT team, design a system for case referrals.
• Serve as liaison with service providers and community partners to ensure APS client needs are met. Provides technical assistance to providers and manages contract and grant funding and approves and monitors budgets and expenditures of state contractors.
• Write issue papers on program outcomes and present them to internal and external stakeholder groups on resource needs of APS clients. Communicate with DAAS Executive leadership on successes and barriers of program implementation and ongoing operations. Participate in committees and community groups, provides education and informational materials.

• Principles of policy formation, analysis and evaluation, service delivery practices, and needs of clientele.

• Program administration, management, and coordination of services and projects.
• Preparation and timeliness of projects, meetings, submittal of grants, and monitoring of contracts.

• Use a variety of computer software packages (I.e. Excel, Word, Outlook, Power-point, etc.).
Ability to:
• Organize and manage a complex and varied workload.
• Conduct research and analysis of facts and propose appropriate resolutions.
• Work with a diverse group of colleagues, network partners, volunteers, etc.
• Develop, test, and implement changes essential for continuous improvement.
The ideal candidate for this position will have:
• Bachelor's Degree in social work, public health, political science, gerontology, communications, business, or related field required.
• At least one year of experience in social work or social services doing case planning or investigations, OR one year in non-profit or government procurement, finance, auditing or policy.
• If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver’s license, complete all required training, and successfully pass all necessary driver’s license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to driver’s license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operates a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) R2-10-207.11).
• All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).
• Successfully pass background and reference checks; employment is contingent upon completion of the above-mentioned process and the agency's ability to reasonably accommodate any restrictions
• A.R.S 41-1969 Each employee or contractor of the Department of Economic Security who has access to federal tax information shall have a valid, unrestricted Level One fingerprint clearance card issues pursuant to A.R.S. 41-1758.07.
