Your Specific Responsibilities:

The Community Planner position is responsible for leading activities relating to the implementation and evaluation of initiatives, and activities designed to promote healthy behaviors, including but not limited to sustaining the departments accreditation status, the strategic planning process, quality improvement initiatives, maintaining and monitoring the Performance Management System (i.e. InsightVision or other), writing plans, processes, procedures, collecting and analyzing data, and other documents, as related. In addition, this position will assist and participate in activities related to the Community Health Improvement Plan (CHIP), and the Community Health Assessment (CHA). This full-time position is under supervision of the Program Administrator and must be able to work independently.

PHAB Accreditation:
- Lead and sustain re-accreditation of Public Health Accreditation Board (PHAB) status.
- Ensure timely submission of documentation for accreditation and re-accreditation.
- Prepare for possible operational site visits.
- Responsible for the preparation of all associated required reports.

Strategic Planning:
- Plan, coordinate, and lead the Strategic Planning process.
- Ensure alignment with the QI Plan, CHIP, CHA and department’s budget.
- Lead teams in developing plans to align the department’s objectives with local, state and federal priorities and initiatives.
- Measure, prepare, and report progress results.

Quality Improvement:
- Lead quality improvement initiatives.
- Work with supervisors to identify opportunities for quality improvement.
- Provide guidance on the use of the PDCA methodology and completion of projects.
- Conduct trainings and exercises, as assigned.
- Coordinate with local and state health department’s quality improvement initiatives to ensure alignment and compliance.

Performance Management:
- Utilize the organization’s performance management system for monitoring and reporting.
- Conduct staff training on utilization of the Performance Management System.
- Lead, coordinate, and prepare all necessary documents (i.e., agenda, reports, presentations, minutes) for the Performance Management Council (PMC) meetings.
- Ensure plan and annual update meets PHAB requirements.

Community Collaboration:
- Participate in various functions, meetings, and activities to ensure DOH representation, participation, and collaboration with community partners.
- Serve on councils, boards, and task forces as needed.

Data Analysis
- Research, review and interpret health data to inform plans, initiatives, and decisions.
- Gather information from various sources, interpret patterns and trends.
- Prepare ad-hoc reports for multiple data and information request.

Other Duties:
- Attend team meetings, in-services, and trainings that support professional development
- Design, distribute, collect, and analyze results of surveys for various functions.
- Ability to organize preparation of written and oral reports and presentations that communicate necessary information to professional audiences, policymakers, and the public
- Ability to work in a culturally competent manner and establish relationships with groups of special concern (e.g., disadvantaged or minority groups, groups subject to health disparities, historically underrepresented groups)
- Ability to manage multiple tasks and competing deadlines

Required Knowledge, Skills, and Abilities:
- Advanced knowledge of public health program evaluation
- Advanced knowledge of data collection and analysis methodology
- Ability to take initiative in performing current job duties and taking on duties of greater responsibility
- Skill in oral and written communication, including the ability to draft, proof, and construct professional documents and correspondence using contemporary business standards
- Strong public speaking and presentation skills
- Proficiency in the use of Microsoft Windows-based business software applications including Microsoft Word, PowerPoint, Excel, and Publisher
- Advanced knowledge of performance management systems
- Strong organizational skills with the ability to multitask and manage multiple projects at the same time
- Ability to establish and maintain effective working relationships with others including the ability to effectively manage conflict

- Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Qualifications:
- Must have a valid Florida driver’s license to perform duties
- Bachelor’s degree from a U.S accredited college or university is preferred
- Work experience in strategic planning, quality improvement, public health, or related field is preferred
