The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Polk County in its mission to protect, promote, and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation. The professional in this position will serve on the Performance Planning Team. The incumbent is responsible for coordinating all Health Equity related programs, plans and functions of the Florida Department of Health in Polk County. This position will assist DOH-Polk staff and local health system partners in community health improvement efforts. The incumbent will work closely with the other Performance Plan managers within the department to help ensure the agency is meeting the established performance metrics.
Your Specific Responsibilities:
Internal Health Equity Team
- Lead CHD Health Equity Team consisting of staff from various program areas to focus on strengthening the internal infrastructure of the CHD to support HE efforts.
- Plan and facilitate regular Health Equity Team meetings
- Provide departmental trainings to other divisions within the health department on health equity topics
- Ensure department documents are inclusive in language and intent
- Pursue diversity in all policies
- Participate in the Office of Minority Health monthly Minority Health and Health Equity Liaison meeting.
- Develop strategies to promote DOH services in underserved communities
- Host a promotional event in the month of April to raise awareness for National Minority Health Month.
- Serves as a representative to the agency’s Performance Management Council and Workforce Development Team.
County Health Equity Coalition

Establish a county Health Equity Coalition and create a HE Plan Taskforce to develop a county HE Plan.

Monitor progress of the implementation of the HE Plan and submit quarterly reports to Central Office.

Report on HE plan progress to the Community Health Improvement Plan

Develop and maintain a comprehensive list of partners serving minority populations and other vulnerable populations

Regularly disseminate information to the appropriate partners regarding training, resources, events, grant opportunities, etc.

Maintain documentation of disseminated resources

Attend monthly statewide COVID Health Disparities virtual meetings.

Identify and coordinate with county-level maternal and infant, HIV/AIDS, chronic disease and health promotion programs, etc., as needed to support the implementation of the HE plan.
Qualifications:
- High School Diploma
- Valid Florida Driver's License
