What are the responsibilities and job description for the PSH Case Manager position at Stephen Center?
Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.
Purpose: The case manager assists Stephen Center client’s to address their barriers in securing and maintaining safe, sanitary, and adequate housing. Supportive services may include physical and mental health services, housing navigation services, employment assistance, legal service referral, services for children, and/or other supports. The case manager works closely with the PSH Manager and community agencies and services to provide resources to help client’s meet their needs. Documentation of all client assessments, services provided, and outcomes is essential to proficient job performance.
Key Accountabilities:
- Ensure all safety guidelines are followed.
- Works as a team member to provide trauma informed care.
- Prepares individual service plans for each admission within 72 hours of intake, consulting with the managers and others as needed.
- Follows through with implementation of the service plan, working with the client and community resources. Formally documents all actions taken, client involvement, outcomes, problems, etc.
- Meets with the PSH Manager to consult on client service plans and to keep them informed regarding client progress.
- Upon successful completion of service plan or when clients leave the Stephen Center, finalizes the service plan record and files for future reference.
- Develops an understanding of the roles and challenges of other Stephen Center staff members and key volunteers and the daily operation of the Shelter and PSH to work effectively within that framework.
- Other duties as assigned.
Position Requirements:
- Ability to two way verbally communicate.
- Human Services, Social Work, or a related degree is preferred.
- Some knowledge of and work with community.
- Knowledge of agencies and community resources also desirable.
- Proficiency in Microsoft Office applications.
- Proficiency in Spanish, a plus.
- Requires the ability to become assertive in the enforcement of organizational rules, regulations and policies of Stephen Center.
- Ability to work collaboratively as a member of a multi-disciplinary team.
- Strong organizational, planning and problem-solving skills.
- Ability to use independent judgment, manage multiple priorities, and exercise discretion in the delivery of services with moderate to minimal supervision.
- Demonstrated ability to communicate effectively, both verbally and in writing.
- Ability to maintain effective working relationships with diverse individuals and groups.
- Conduct self in a manner of professionalism that is conducive to recovery from mental illness, co-occurring disorders and overall wellness, including a minimum of one year of continuous sobriety if in recovery, from the substance for which you are in recovery.
