As a Stonerise Nurse Liaison, you will be responsible for facilitating admissions from assigned hospitals, clinics and MD offices into the Stonerise Network of skilled nursing centers and home health agencies. You will clinically review and determine admissions criteria, market Stonerise services, develop professional relationships and identify opportunities for growth and improvement. You will make patient acceptance and placement decisions on behalf of Stonerise and communicate decisions to referral sources and centers. Some of your primary responsibilities include:

As a Stonerise Nurse Liaison, you will be responsible for facilitating admissions from assigned hospitals, clinics and MD offices into the Stonerise Network of skilled nursing centers and home health agencies. You will clinically review and determine admissions criteria, market Stonerise services, develop professional relationships and identify opportunities for growth and improvement. You will make patient acceptance and placement decisions on behalf of Stonerise and communicate decisions to referral sources and centers. Some of your primary responsibilities include:
- Communicating with the interdisciplinary team daily to ascertain information and knowledge about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events.
- Managing electronic referral systems for assigned centers and developing professional relationships with case managers and discharge planners, hospital employees and management, patients, and their families.
- Completing clinical reviews via Allscripts and/or onsite evaluation as necessary or requested and coordinating a seamless transition to assigned centers.
- Marketing Stonerise care centers and home health to the patients and families, hospital employees, and the healthcare industry.
- Remaining visible at assigned hospitals and being available as a resource to promote Stonerise and affiliates in a positive manner.

Education, Licensure Certification Requirements
- Two (2) years of experience in sales and marketing
- Active WV RN license.
