Required Qualifications (as evidenced by an attached resume):
Bachelor's degree (foreign equivalent or higher) in health related field. In lieu of a Bachelor's degree, Four (4) years of full-time experience working in case management and/or working with people living with HIV may be considered. Experience engaging in case management activities such as assessing, planning, implementing, monitoring and evaluating patients or experience with client's health and human services.
Preferred Qualifications:
Experience working with and generating reports in data management systems. Experience working with clinical case management. Experience with quality management. Bilingual in English/Spanish.
Brief Description of duties:
The Education Specialist will provide case management to people living with HIV as well as those at risk. The Education Specialist will assist the division's funded programs with data management and report generating to ensure program standards are met.
Duties:
• Provide medical case management to include intake assessments, service plan development/updates, concrete supportive services and referrals to people living with HIV and those at risk.
• Coordinate client inreach/outreach
• Provide risk assessment/reduction services, crisis intervention and HIV counseling
• Assist with PrEP outreach, education, referrals, linkage to care and medical adherence.
• Assist with quality improvement activities including data management and generating reports to evaluate program progress and quality goals.
• Other duties as assigned by division administrator as appropriate to rank and departmental mission.

The Research Foundation of SUNY is a private educational corporation. Employment is subject to the Research Foundation policies and procedures, sponsor guidelines and the availability of funding. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.

The starting salary range (or hiring range) to be offered for this position is noted below, it represents SBU's good faith and reasonable estimate of the range of possible compensation at the time of posting.
