AVAILABLE SHIFTS
Overnight Caregiver (12am-8am) Fri/Sat/Sun
Evenings (4-12pm)
Weekends Sa/Sun(11am-7pm)
General Function
Transition Support Workers create a safe and nurturing home environment and are responsible for residents’ physical well-being and supporting their self-sufficiency skills development.
Primary Responsibilities
· Documentation/Paperwork: Maintain on-shift documentation in each resident’s electronic files, and other paper or electronic forms to document resident needs, services, or behavior.
· Resident Supervision: Provide a safe and nurturing environment for all Safe Harbor residents. The activities that a Transition Support Worker is responsible for is determined by the shift worked but may include: general oversight of the resident living areas, supervision of residents while they are in the home, tracking of students signing in/out of the home.
· Transition Support Workers will support residents in learning daily life tasks that residents are assigned involving shopping, meals, hygiene, decision making, social interactions, safety, etc.
· Cooking/Housecleaning: Supervise cooking and housecleaning assigned to residents, instruct residents in cooking or cleaning tasks as needed, perform other cooking/housecleaning duties as assigned by the director. If there has been a discharge on your shift, the room and bedding should be cleaned to prepare for the next resident.
· Transportation: Provide resident transportation to/from classes or educational activities, appointments, employment, and social service appointments as needed.
· Communication: Transition Support Workers will communicate to the on-coming staff any resident changes, needs and behaviors. Immediately report any incident of resident or staff abuse to the Program Director or his/her designee. Report inappropriate behavior that violates house rules to the House Parent or Program Director.
Related Duties:
- Work with other staff to coordinate resident needs or complete documentation.
- Attend and participate in staff meetings and trainings
- Perform all other duties as assigned by the Program Director or designee
Special Requirements:
- Light lifting up to 25 pounds
- Basic mobility to accompany residents when needed
- Reaching/movement involved for domestic tasks
- Obtain and maintain adult first aid and CPR certifications
Qualifications
· Minimum of high school diploma or equivalent
· Two years of experience in child care, youth development, education, social services, or related care
· Pass all necessary background and drug tests
Reports to: HSP and/or Transition Program Director

Pay: From $15.00 per hour
Schedule:
- 8 hour shift
