Hospice Nurse (PRN) *EAST VALLEY

We are currently looking for a motivated part-time or PRN Hospice RN to supplement patient visits in the East Valley. We are looking for a Per Diem, PRN or Part-time nurse (RN) who would be available to take visits as they arise (covering PTO, sick time, and when case managers acuity is maxed) in Gilbert, Chandler, Sun Lakes, and Mesa. PLEASE NOTE: Indeed does not have a "per visit" option. $75 isn't the hourly rate but the per visit rate for routine follow-up visits.

Utilizing their depth of experience and knowledge, PRN hospice nurses are responsible for:

• Completing hospice evaluations as requested (admission if comfortable and training would be provided)
• Conducting in home visits as assigned; assessing the patient’s condition, and teaching the patient and family
• Developing care plans based on diagnosis and needs
• Administering medications and treatments as prescribed by physician
• Communicating patient needs and changes with the team as necessary

Requirements:

• Valid AZ RN license (multi-state compact license is acceptable as well) (required)
• Minimum of two years of Hospice nursing experience (required)
• Valid AZ Driver’s License and automobile insurance (required) with reliable transportation (required)
• Pass background check (required)
