Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full time hybrid Treatment Adherence RN to support our Genesis program in Patchogue, Amityville, and Brentwood, NY.
This position provides nursing care, medical case management, adherence education, support, monitoring and follow up services to HIV positive patients. Will render nursing care to patients as directed by the medical provider pursuant to the protocols of HRHCare and Works cooperatively and collaboratively with the entire Genesis and medical teams in various sites.
Essential Functions:
- Identifies and enrolls patients eligible for the PAWH program.
- Conducts intake, comprehensive assessments/re-assessments and evaluations for PAWH patients.
- Develops and monitors care plans for PAWH patients.
- Develops linkages and refers patients for additional services.
- Provides appropriate follow up on patient referrals, missed appointments etc.
- Provides health education and risk reduction to patients within scope of discipline to assist in patient engagement and retention and reduce barriers to care to enhance health outcomes.
- Develops and implements retention in care strategies.
- Provides ongoing follow up and support to patients who are not virally suppressed.
- Participates in and/or facilitates case conferences.
- Maintains data and case records as required and prepares necessary reports.
- Participates in ongoing training and regional collaborative s to enhance knowledge and skills.
- Participates in Quality improvement trainings and projects.
- Works collaboratively with entire Genesis team.
- Implements evidence-based programs to improve health outcomes of patients.
Education/Experience:
- Graduate of an accredited nursing program
- Active NYS RN Licensure
- Bachelor's degree preferred
- 2-4 years experience preferred

Pay rate $41.00 - $46.70 ph
