What are the responsibilities and job description for the Medical Assistant/Patient Navigator position at Sun River Health?

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Medical Assistant/Patient Navigator to support our Peekskill site.
Provides recovery support to reduce barriers of care and shares strategies to improve patient health outcomes. This includes educating patients regarding MAT, HCV, risk reduction, medical and medication adherence. Provides education resources and materials that serve to promote recovery support education on harm reduction, MAT, Naloxone, and treatment. Provides Harm reduction and opioid overdose and prevention training and support.
ESSENTIAL FUNCTIONS:
- Prepare exam room and any needed equipment and assists/chaperones during examination and treatment of patients.
- Provide basic instruction/health education to clarify provider instructions, procedures and referral needs.
- Facilitates patient care within different departments of SRH and outside agencies.
- Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
- Perform in-house labs, collect specimens for out-side labs (ex. Pap, cultures, etc.) perform quality control in accordance with organizational protocols.
- Keeps clinical area stocked and in good order. Assures that all dated material is kept current and disposes of outdated supplies.
- Responsible for referring patients to appropriate agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.
- Takes vital signs, vision, and/or hearing tests, record patient data in eCW, (laboratory results, pain level, PEAS, PhQ, Smoke status, etc.), Document reason for visit and insure CVRs are completed as needed.
- Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.
- Ensures all appropriate logs are maintained.
Minimum Education Requirement: High School diploma/ GED with Medical Assistant Training Certificate
Preferred Education Requirement: Associate's degree with Nationally Certified Medical Assistant Certificate
Salary Range: $19.00- 22.00
