SUMMARY:
The Social Worker-Bachelor’s Level position provides linkages to community resources and assists with concrete tasks such as transition planning, discharge planning, forms completion and assistance with financial resources, specifically Medicaid initial applications and annual re-determinations.
POSITION QUALIFICATIONS/REQUIREMENTS:
• Education: Baccalaureate degree of Social Work from a School of Social Work accredited by the Council on Social Work Education
• Experience: One year of experience with the geriatric population preferred; one year of experience in Medicaid work preferred

JOB DUTIES/RESPONSIBILITIES:
1. Ensures participant/family has access to financial resources including information about Medicare and Medicaid.
2. Assists in the Medicaid initial application and annual re-determination process for participants.
3. Collaborates with the IDT to help facilitate with transfer planning and coordination of care as patients move among care settings. Completes forms necessary for changes to level of care and transitional changes.
4. Participates in interdisciplinary team meetings as indicated.
5. Maintains electronic clinical record and follows organizational guidelines for completing documentation.
6. Seeks professional supervision from team leadership/supervisor on challenging cases.
7. Makes appropriate referrals to interagency programs and community services.
8. Educates the community regarding Empath Health philosophy of care through case by case consultation and to the care community through outreach activities. Serves as a resource to the care community and provide outreach.
9. Provides other services as requested by team leadership.
