JOB TITLE: Medical Records Clerk
RESPONSIBLE TO: Medical Records Supervisor MAIN FUNCTION: To file and maintain all patient clinical records to ensure all Agency goals and objectives are met.
DUTIES AND RESPONSIBILITIES
1. Prepares 10 cert and 10 recert packets for RN admissions and re-certifications at all times. Keeps cubbies stocked with proper forms.
2. After QA reviews and RN signs the cert., the Medical Records Clerk prepares a chart and field chart(s), makes copies of 485's (Scheduler, Billing Clerk, QA, and MSW if needed).
3. After QA reviews and RN signs the recert, Medical Records Clerk then updates chart and field chart(s), then distributes copies of 485's (Scheduler, Billing Clerk, QA, and MSW if needed).
4. Assist when needed to disassemble each active chart and establishes an inactive chart after all discharge procedures have been completed.
5. Assist when needed on maintaining MSW referral log which includes patient name, patient number, date referral was given to MSW and date MSW reports are received.
6. Assist when needed with making copy of Admission Data Base and attaches it to the MSW requisition form which is then placed in the appropriate place for MSW clerk to pick up.
7. Assist when needed in logging in all MSW assessments and follow-ups, in MSW log, as they are turned in.
8. Assist with faxing copies of all summary report to physicians after signed and dated by the appropriate disciplines, and lab reports to M.D. with stamp that indicates M.D.'s copy.
9. Print 485’s for field nurses to file in field charts.
10. Files copies of lab reports and carbons of the single patient medication sheets in field charts.
11. Attaches allergy stickers to patient's field and permanent charts on the front below the patient's name and number. Attaches confidential stickers on field charts in upper right-hand corner.
12. Files and/or uploads all labs, fax orders, therapy notes, wellness checks and admitting information into patient chart and/or electronic medical record.
13. Assist when needed in reviewing incoming copies of Telephone orders from scheduling to look for discharged patients, MSW freq., or HHA information.
14. At the end of the year, medical records clerk is responsible for accounting for every chart, boxing and storing them.
15. Faxes D/C and transfers to doctor’s offices and records into logbook and will maintain transmittal sheet for verification.
16. Ensures that clinical records are kept confidential taking note of all outgoing charts and who they're being checked out to.
17. Keeps working area and file cabinets in an orderly manner.
18. Any other duties deemed necessary by the supervisor/Administrator.
Job Type: Full-time
Pay: $10.00 - $14.00 per hour
