Position Summary
The Social Services Assistant provides assistance to the Director of Social Services to include but not limited to admission of all short-term residents, discharge planning for all short-term residents/family meetings, MDS’s, Care Plans, and room changes for all short-term residents. Process ULTC 100’s for transition to Long Term/ALF/Community, PASRS and PETI’s for all residents in-house in a timely manner.
Essential Duties
- Process admissions and MDS’s for all short-term residents.
- Participates in Care Plans for all short-term residents.
- Participates in all short-term resident family meetings.
- Discharge planning for all short-term residents/family meetings.
- Handle PETI’s for all residents in-house.
- Must attend Medicare Staffing meeting every week.
- Process room changes.
- Process all PASRS no later than five days after resident’s arrival.
- Process ULTC 100’s for transition to Long Term/ALF/Community.
- Supports Director of Social Services with all tasks for short-term residents.
Basic Qualifications & Experience
- Must possess basic knowledge of social service needs/systems.
- Must be knowledgeable of Medicare/Medicaid programs and regulations.
- Must possess effective communication skills.
- Must be familiar with a patient’s rights and responsibilities.
- Must be able to plan and coordinate resources to meet residents’ needs.
- Must be able to work a varied schedule including holidays and weekends if needed.
- Must have the ability to establish and maintain effective working relationships with physicians, residents’ families, community resources and all employees in the facility.
- Must possess knowledge of Transportation procedures for emergency transport situations.

The base range represents the low and high end of the pay range for this position. Actual pay will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. The range listed is just one component of our total compensation package for employees.
