At SYNERGY HomeCare we provide non-medical services that allow clients to live more independent lives at home – wherever home may be. The RN will report directly to the franchise owner. The RN is responsible for ensuring compliance with state guidelines and understanding company policies and procedures for office and field operations. This position includes training caregivers, home assessments, ensuring client information is documented and that cares provided fit the needs of our client.
We are looking for the right person who can help maintain and grow a new business as well as work with the team to increase our client census. A vital component of this position is to communicate the agency mission and philosophy and ensure that our clients and family members better understand the full potential of our services. The successful candidate will also be responsible for collaborating with the owner to improve client support, enhance client satisfaction and maintain healthy care provider morale. We are looking for an energetic professional who enjoys wearing multiple hats.
Duties and Responsibilities:
- Initiate and Complete HomeCare Assessments and Reassessments for Comprehensive Clients
- Plan, Develop and Implement Plan of Care
- Evaluate Plan of Care effectiveness, modify as needed
- Quality Assurance
- Coordinate and Collaborate with various Disciplines, Physicians, Communities, Peers, Client families, Office Staff, and Medical Entities
- Training of Staff, Availability to answer on-call questions
Criteria:
- Must possess Critical Thinking and Reasoning Skills
- Complete all Documentation in Accordance with MN State Laws and Nursing Board
- Ensure Compliance with local, state and Federal laws
- Must possess Interpersonal Skills
- Positive Attitude
- Self-Motivated and Strategic Decision Maker
- Willing to travel in East Central MN and West Central WI
Required Qualifications:
- Current MN State RN License (license in WI a plus)
- TB negative test, pass fingerprinting requirements
