Responsibilities
POSITION SUMMARY: The Hospice Bereavement Coordinator is responsible to plan, implement and maintain a Bereavement program to meet the needs of families/caregivers with Tabitha Hospice for up to 18 months following clients death. The Bereavement Coordinator is also responsible for providing supportive services to Hospice clients and families while delivering quality service and promoting good public relations. ESSENTIAL FUNCTIONS:
- Develops, coordinates and implements Hospice of Tabitha’s Bereavement Program.
- Coordinates services to include the provision of the assessment and delivery of grief counseling needs and services (one-to-one, groups, mailing program and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers for up to 18 months following client's death.
- Responsible for the delivery of bereavement services, as assigned.
- Documents all client contacts to meet state, Federal and agency standards.
- Provides crisis intervention for Hospice or community bereaved.
- Assists as needed in evaluating and planning services to meet Hospice personnel support needs, i.e., yearly retreats, staff bereavement, team building, support groups and general care.
- Serves on Performance Improvement Committee, as requested.
- Provides education and support to Hospice volunteers.
- Participates as team member to ensure that the department work loads and goals are met.
- Willingly takes on additional duties and/or assignments, assisting with projects and responding to requests as assigned or as necessary.

Qualifications
COMMUNICATION: Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job. Sufficient fluency to be able to understand and follow written and verbal instructions, provide instructions to others, read and understand policies, procedures or other similar documentation, interact successfully with Seniors/clients/caregivers, hold a conversation, conduct or participate in meetings, compose professional e-mails or other written communication, prepare legible, accurate and thorough documentation as required by the job and effectively interact with TEAMembers,
PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment, including but not limited to the equipment described below. Ability to move about the work area. Must have unrestricted lift/push/pull ability and pass a 25 - pound physical exam demonstrating strength and agility prior to employment and from time to time thereafter at Tabitha’s discretion.
EQUIPMENT USAGE: Standard and specialized equipment including PC, photocopier, scanner, fax machine, telephone and Microsoft Office Suite.
AGE REQUIREMENT: At least age 18.
EDUCATION: Bachelor’s Degree in Social Work, Human Services, Theology, Counseling or similar field required. Certified by the State of Nebraska as a bachelor’s level social worker preferred.
EXPERIENCE: Clinical experience appropriate to the counseling and casework needs of the terminally ill preferred. Training and/or experience in dealing with grief required.
CREDENTIALS: Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. CPR Required. CPR Certification which must be kept up to date at the expense of the employee. Must follow Tabitha’s vaccination requirements.
