What are the responsibilities and job description for the Pharmacy Intern position at Tandem Health?
Pharmacy Intern Duties and Responsibilities:
- Prepare patients for clinical pharmacy services such as Medicare annual wellness visits and diabetes self-management education
- Schedule appointments for clinical pharmacy services
- Perform medication therapy management and chronic care management for retail and clinical pharmacy operations
- Provide patient education and counseling
- Provide provider education
- Enroll eligible patients into patient assistance programs
- Order continuous glucose monitoring supplies
- Enroll patients into programs such as LibreView and Dexcom Clarity for continuous glucose monitoring report interpretation
- Assist in program enrollment for compliance packaging, medication synchronization, and delivery
- Assist in the completion of Prior Authorizations
- Assist pharmacists and pharmacy technicians with retail pharmacy operations in accordance with state and federal regulations
- Complete training requirements to stay up to date on policies and procedures for the 340B drug program, as well as any drug assistance programs in which the pharmacy participates
- Follow all policies and procedures related to the 340B drug program, as well as any drug assistance programs in which the pharmacy participates
- Maintain an awareness of developments in the community and pharmaceutical fields that relate to job responsibilities and integrates them into job responsibilities
- Staff in Tandem Health Pharmacies as a traditional retail pharmacy intern as needed
Qualifications:
- Must have successfully completed High School or GED equivalent.
- Must be accepted, attending or newly graduated from an accredited college of pharmacy
- Must possess intern certification from South Carolina Board of Pharmacy
