A healthcare services organization in New York is actively seeking a licensed Registered Nurse (RN) to join their team in Buffalo as their new Hospice Nurse. In this role, the Hospice Nurse (RN) will be responsible for providing professional nursing services for individualized patient care and psychosocial / spiritual interventions for assigned patients / families.

The Hospice Nurse (RN) will:
- Perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness
- Perform pain assessments using Edmonton Scale
- Organize and prioritize assignments to consistently meet productivity standards and complete documentation requirements within identified time parameters
- Appropriately explain duties and functions of the interdisciplinary team
- Modify the plan of care timely and appropriately in anticipating problems that may cause or create a threat to patient safety or lack of symptom control
- Maintain the security of patient electronic / written records
- Perform other duties, as needed
Qualifications:
- Graduate of an accredited School of Nursing
- Current NYS Registered Nurse (RN) licensure
- Working knowledge of Electronic Medical Records (EMR) systems
- Microsoft Office Suite proficient (Excel, Outlook, PowerPoint, Word, etc.)
- Solid problem solving and time management skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- 1 year of professional Nursing experience
