What are the responsibilities and job description for the Nurse Navigator position at Target Health?

The National Hypertension Control Initiative (NHCI) Nurse Navigator position is responsible for carrying out activities for assigned primary care health centers participating in the NHCI program.  This position is also responsible for providing hands-on field consultation, coaching and technical support to ensure overall program success.

Must be able to travel. This is a full-time position.

Responsibilities
·        Work with assigned primary care health centers to implement and support strategies related to the National Hypertension Control Initiative (NHCI).

·        Cultivate relationships with clinical leads at participating health centers. Provide technical assistance through webinars, quality improvement activities, and one-on-one clinical practice facilitation to support improvement of hypertension management and control

·        Perform systematic assessments of current clinical practice and identification of areas for improvement based on practice data, provider/care team knowledge and skills, clinical workflow processes and available resources

·        Develop, communicate, and execute quality improvement operational plan

·        Provide targeted training and technical assistance to improve blood pressure control using the MAP ( M easure accurately, A ct rapidly, P artner with patients) Framework and Self-Measured Blood Pressure (SMBP).

·        Responsible for providing evidence-based educational resources, practice change facilitation, and quality improvement strategies needed to adopt SMBP practice and improve blood pressure control within the patient population participating to effectively achieve the goals of the National Hypertension Control Initiative

·        Ensure assigned organizations have access to the breadth and depth of resources available from the AHA by leveraging resources, tools and national center assets (e.g., communication, science, advocacy, field resources). Utilize existing AHA materials, products and recruitment tools or develop new tools and materials as needed. 

·        Establish, collect, and report on measurements of success, such as program participant data, trends, and improvement or control of hypertension. 

·        Support evaluation and tracking of grant deliverables and contractual obligations

·        Collaborate with the AHA NHCI on program initiatives
 
·        Communicate through virtual meetings and collaboratives

·        Provide resources and support training for technical aspects of grant activities related to blood pressure measurement devices, connectivity between devices and electronic health records, and data reporting and analysis

·        Identify, monitor progress, and share common problems/solutions across sites to leverage shared learning

·        Seek additional expertise and resources as needed

Qualifications
·        Licensed LPN or RN

·        Bachelors’ degree in Health Sciences, Business, Healthcare Administration, Communication, Marketing, related field, or equivalent experience.

·        Five (5) – Eight (8) years of the following experience

·        Experience working in healthcare delivery with operational knowledge of primary care practice environments and/or ambulatory clinic workflow

·        Clinical knowledge/experience working with clinical practice guidelines and management of chronic health conditions; preferably hypertension. Focus to include aspects of high blood pressure management: measurement, treatment intensification, medication management, and patient engagement and adherence strategies

·        Experience/training in practice facilitation

·        Experience leading a team

·        Quality improvement knowledge/skills regarding various methodologies (e.g., Lean, Six Sigma, PDSA, TPS) and comfort analyzing data for performance improvement opportunities

·        Strong interpersonal skills and the ability to work effectively with staff and others as part of a team.

·        Demonstrated ability to simultaneously manage multiple, complex projects in varying stages of development under time constraints.

·        Demonstrated understanding and appreciation for the use of technology and information systems.

·        Exceptional oral and written communications skills.

·        Must be able to travel.

·        Basic knowledge and understanding of healthcare information technology (IT) such as electronic medical records, or other related systems or database functions

·        Ability to analyze and interpret technical requirements to improve data sharing and integration

·        Project management skills and the ability to influence performance without direct line accountability

·        Educational skills to structure group training for adult learners and team-based caregivers

·        Strong interpersonal and relationship building skills

·        Excellent oral and written communication

·        Comprehensive Microsoft Office suite skills including Word, Excel, PowerPoint, and Outlook

·        Working Knowledge of cardiovascular health, cardiovascular disease (CVD) and associated risk factors, complications, and management

·        Bilingual, English/Spanish.

Work Hours: Full Time position.  Monday through Friday. Work will be possible remote and onsite at the office.  All work supplies/equipment will be provided.
