Summary:
Provides health related assessments and medical case management services to adults with autism and developmental disabilities living in TERI ICF/DD-H licensed residential group homes in North San Diego County.
Essential Duties and Responsibilities:
- Assessing, Observing, and speaking to residents.
- Recording details and symptoms of patient medical history and current health.
- Creating, implementing, and evaluating resident care plans with medical team.
- Providing case management support and oversight.
- Attending coordinated client service planning meetings.
- Maintaining the highest standard of care.
Salary Range: $ 39.00 – 40.00 / Hour
Full time Monday – Friday with monthly on call weekend requirement.
Qualifications:
- Current CA RN License.
- Experience with people with developmental disabilities preferred.
- Previously provided in-home care, individual or group.
- Ability to lift and transfer people when needed.
- California Driver’s license and good driving history
- Willingness to complete a drug test.
