Terros Health is pleased to share an exciting and rewarding opportunity for a Discharge Planner working at our 27th Avenue Certified Community Behavioral Health Clinic (CCBHC).
Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site Manager.

Terros Health is hiring a Discharge Planner at our 27th Avenue, Certified Community Behavioral Health Clinic (CCBHC) in Phoenix, AZ.

Discharge Planner
- Activities will be performed in both inpatient and outpatient settings.
- Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
- Work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans
- Clinically indicated to ensure effective transition to Outpatient or community services
- Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.

Qualifications
- High School Diploma or GED.
- Must possess a combination of behavioral health education and work experience totaling a minimum of four (4) years;
- A bachelor’s degree in any field, plus one year of work experience in a behavioral health service delivery:
- A bachelor’s degree in a behavioral health field.
- Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
- Must pass a TB Test.

Salary : $2,000
