What are the responsibilities and job description for the MDS Coordinator LPN or RN position at The Blossoms?
We are seeking an experienced MDS Coordinator for our Milford location. This individual must have a vaild RN or LPN license in the State of CT.
Full Time, Part Time and PRN positions available.
Job Duties
· completing minimum data set assessments
· creating comprehensive plans of care after review of the patient’s medical record
· communication with direct care staff, the patient’s physician, and family
· develops and completes the patient assessment process in accordance with the requirements of federal and state regulations and company policies and procedures.
- Must be able to coordinate MDS systems, resident assessment, and care plans for each resident timely.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.

· Strong clinical assessment skills
· Ability to make independent decisions on a regular basis
· Effective interpersonal skills and the ability to work with an interdisciplinary team
· Basic computer skills
· Proven written and oral communication skills
· Proven decision making and analytical skills
Qualifications
- Must possess a current, unencumbered license to practice as an RN/LPN in this state
- Must be able to read, write, speak and understand the English language
· Must have experience in a skilled nursing and rehabilitation setting
· Must posse strong clinical skills
· Must have a strong understanding of the MDS process
· At least 1 year working in a skilled nursing facility setting.
EDUCATION:
Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university, RN preferred.
