What are the responsibilities and job description for the APRN, Primary Care position at The Centers?
Patient Assessments:
- Develops and implements Nursing Plan of Care
- Ensures that physical examination information form/medical database is completed within established timeframes (program specific)
- APRN orders signed within established timeframes
- Conducts and correlates findings of physical assessment
- Demonstrates competency in patient assessments specific to ages of populations served
- Orders nursing interventions which are pertinent to patients' problems/needs
- Orders labs. EKGs, and other tests according to protocolTreatment Plan:
- participates in development of the treatment plan
- Defines measurable and realistic patient outcomes
- Evaluates patients' progression toward expected outcome
- Analyzes specific changes in patient condition, implements interventions and documents responses
Medication Management:
- Prescribes medications judiciously, follow protocols. monitors and manages side effects and reports side effects on adverse drug reaction form
- Consults with and utilizes feedback from physician, nursing/pharmacy/dietician regarding medical/nursing/pharmacy/dietary issues
- Demonstrates competency in medication management specific to ages of population served
Documentation:
- Documents all encounters with patient in the clinical record
- Signs all APRN orders in a timely manner
Committee Participation:
- Attends and patriciates in Medical Staff committee meetings
Quality Improvement Activities:
- Participates actively and responds within appropriate timeless to all QA functions and activities
